This program offers services to individuals who, because of their medical needs, would qualify for nursing home placement, but would rather stay in their own home. The recipients are screened for medical need prior to receiving services.
Services Must Be Certified By A Physician and Include:
- Nursing
- Assistance with bathing, grooming, dressing, feeding, toileting, transfer/ambulation, cleaning. laundry, meal preparation, escort, shopping, and/or assistance with self-administered medications as well as certain tasks delegated by a registered nurse.
- Minor home modifications
- Adaptive Aids/D.M.E.
- Approved medical supplies
